Step 4: Training &

Implementation

Our team will provide on-site training and hands-on support to guide your practice through the PCMH transformation. This includes developing customized PCMH standards, writing protocols, integrating standards into your EMR system, and training your staff (both clinical and administrative).

Step 5: Certification

We will manage the entire certification process, including preparation, documentation, submission, and tracking of the PCMH application, ensuring a smooth and successful outcome.
